Public Health & Health Systems

Strengthen the path from programme intent to equitable access.

GEMBA can support institutions to understand health-system challenges, strengthen programme design and shape practical approaches for delivery. Our focus is on how policies, services, workflows, people and evidence connect - and what must change for programmes to be more accessible, coordinated and usable.

The challenge

Health outcomes depend on the system around the service.

A technically sound intervention can still encounter barriers when roles are unclear, workflows do not fit frontline reality, coordination is fragmented or intended users cannot access and use services. These are not only programme-design issues; they are system and implementation issues.

GEMBA approaches public health with attention to the full delivery environment: policy and programme intent, institutional arrangements, operating data, frontline capacity, community access and the feedback needed to adapt.

This capability may be relevant when an institution is addressing:

  • Gaps between programme design and operational readiness
  • Coordination and workflow barriers across institutions and delivery levels
  • Frontline usability, workload and capacity constraints
  • Access, equity and communication considerations affecting service use
  • Monitoring information that is available but not easily translated into action

How GEMBA can support

Technical and systems support shaped around the requirement.

The final combination of support areas is agreed against the institution's objective, scope, governance, timeline and available resources.

  • Situation and gap analysis

Bring together available data, policy, programme documentation and stakeholder perspectives to clarify the current situation, priority gaps and feasible entry points.

  • Programme and implementation design

Translate objectives into a practical theory of change, delivery model, roles, workflows, milestones, resource assumptions and implementation considerations.

  • Health-system strengthening

Examine how governance, service-delivery arrangements, workforce, information, coordination and accountability influence programme performance and access.

  • Access and equity lens

Consider how geography, gender, vulnerability, information, cost, trust and service experience may shape who can reach and use a programme or service.

  • Stakeholder coordination

Support structured engagement across government, technical institutions, frontline actors and other approved stakeholders around shared decisions and responsibilities.

  • Monitoring, learning and documentation

Define practical measures, review routines and decision-oriented products that help teams track implementation, surface constraints and adapt responsibly.

Our approach

Connect evidence, systems and frontline reality.

The precise approach depends on the assignment, but a health-systems engagement may move through four connected stages.

  1. Step 01

    Understand the system

Clarify the programme objective, institutional landscape, delivery pathway, available evidence and decisions the work must inform.

  2. Step 02

    Locate the barriers

Combine data, document review and stakeholder insight to identify where access, coordination, workflow or implementation is breaking down.

  3. Step 03

    Shape practical action

Co-design recommendations, programme components, roles, tools or support mechanisms that fit approved structures and operating realities.

  4. Step 04

    Support use and learning

Help translate the agreed direction into implementation-ready material, review processes and learning loops within the confirmed scope.

Connected support

Combine capabilities around the requirement.

This capability can be commissioned on its own or connected with other GEMBA capability areas where the assignment requires a more integrated response.

  • Research, Assessments & Evidence

for situation analysis, field inquiry, implementation research and evidence synthesis

  • SBCC & Strategic Communication

for audience insight, service communication, demand-side barriers and programme communication

  • Programme Implementation & Field Operations

for field readiness, capacity building, quality assurance and monitoring support

Experience behind the capability

This capability is informed by Dr Shweta Richa's individually accumulated experience across public-health research, nutrition, health systems and multi-district field operations, together with Amit Rakesh Dubey's experience in public-system communication and programme support. Their former assignments remain individually attributed and are not presented as GEMBA projects.
